Dinner/Dance 2014 invitation and reply form

Hello,

Please find the Dinner/Dance invitation and reply form attached together in one document.

The event has sold out for the last two years so please RSVP as soon as possible! Payments must be made by 13th September, all details are on the invitation.

Accommodation is available at the hotel and they have reserved enough double rooms for everyone with a discounted price for people attending this event. Please book these rooms at your convenience and make sure to use the special code in order to get the reduced rate.

If you do not wish to print off the reply form please email me and send the payment separately, I will keep an updated list on the forum of attendees and those who have paid. If there are any disputes please let me know ASAP!

Cant seem to see any information in the form.  3 blank pages and one line asking for an e-mail address.  Please could you check it.  also *.docx files don't work well on OSX Mac.  could you PDF it or standard word 2003 *.doc for download.
